Reporting Injuries / Incidents

To report an incident, please fill out the Incident Report Form and email it to us.

If you are unable to email this form you can also print this form, sign it, and send it via internal mail to Health and Safety Services in B360 / R112, Nanaimo Campus.

What to Report

All injuries involving employees, students, contractors, and visitors must be reported to Health and Safety Services in accordance with WorkSafeBC Regulations. A Workers compensation claim may also have to be filed .

Health and Safety Services is the central reception point for all incidents at VIU therefore incidents such as theft, vandalism, property damage or property loss or any incident involving student or employee conduct must be reported immediately.

Any vehicle incidents must be reported immediately.
